Executive Health

Our Executive Health Program provides a complete range of medical services conveniently all in one location. Services offered through this program include men's and women's executive health packages, spa services and more.

Social Determinants of Health

Our social determinants of health program, HMH Healthy Connections, is a three-pronged strategy designed to foster sustainable investments in underserved and marginalized populations through resources beyond traditional health care to meet their varied, whole-person needs. Its focus is on three primary areas:

  • Healthy Connections CHW Initiative - A community health worker-based intervention to address social determinants in targeted patient populations and communities using trained CHWs to provide direct support via in-person and virtual interactions.
  • Healthy Connections SDOH Initiative - A five-pronged anchoring strategy addressing food security, housing stability, transportation issues, caregiver support and mental health/behavioral health/SUD across the patient, VBC and geographic communities.
  • Healthy Connections SDOH Documentation Screener - A network-wide SDOH data collection effort focused on screening for and identifying SDOH needs across all patient Electronic Health Records


Our telehealth program, HMH NOW, offers 24/7 video visits with a doctor. No appointments are necessary, and prescriptions can be sent to the pharmacy of your choice. Treated symptoms include but are not limited to:

  • Allergies
  • Asthma
  • Bronchitis
  • Colds and flu
  • Earache
  • Headache
  • Hives
  • Insect bites
  • Pink eye
  • Rashes
